The prognostic importance of chronic end-stage diseases in geriatric patients admitted to 163 Italian ICUs

BACKGROUND: The number of elderly patients undergoing major surgical interventions and then needing admission to intensive care unit (ICU) grows steadily. We investigated this issue in a cohort of 232,278 patients admitted in five years (2011-2015) to 163 Italian general ICUs. METHODS: Surgical patients older than 75 registered in the GiViTI MargheritaPROSAFE project were analyzed.

Changes in electromyographic amplitudes but not latencies occur with endotracheal tube malpositioning during intraoperative monitoring for thyroid surgery: Implications for guidelines

Objectives/Hypothesis: During intraoperative neural monitoring (IONM) in thyroid and parathyroid surgery, endotracheal
(ET) tube migration can result in a decrease in vocalis electromyographic (EMG) amplitude without a concordant latency
elevation during stimulation of the recurrent laryngeal nerve (RLN).
Study Design: Retrospective review.
Methods: Data were reviewed retrospectively for thyroid and parathyroid surgery patients with IONM of the laryngeal

Letter regarding 'Covered versus uncovered metal stents for malignant gastric outlet obstruction. Systematic review and meta-analysis'

We read with interest the paper by Hamada et al.1 (covered vs uncovered metal stents for malignant gastric outlet obstruction. Systematic review and metaanalysis”).
We prospectively analyzed the clinical outcomes of 72 patients2,3 and we came to similar conclusions. Conceptually, covered stents present the advantage of preventing tumor ingrowth within the stent and, being more compliant, they could facilitate pyloric motility. However, they dislodge
more easily, and distal migration of the stent can lead to serious consequences ...

An alternative treatment strategy for complicated chronic wounds. Negative pressure therapy over mesh skin graft

Extensive skin defect represents a real problem and major challenge in plastic and reconstructive surgery. On one hand, skin grafts offer a practical method to deal with skin defects despite their unsuitability for several complicated wounds. On the other hand, negative pressure wound therapy (NPWT), applied before skin grafting, promotes granulation tissue growth. The aim of the study is to evaluate the improvement in wound healing given by the merger of these two different approaches. We treated 23 patients for large wounds of multiple factors.

Role of working memory, inhibition, and fluid intelligence in the performance of the Tower of London task

We investigated the relationship between verbal and visuo-spatial measures of working memory, inhibition, fluid intelligence and the performance on the Tower of London (ToL) task in a large sample of 830 healthy participants aged between 18 and 71 years. We found that fluid intelligence and visuo-spatial working memory accounted for a significant variance in the ToL task, while performances on verbal working memory and on the Stroop Test were not predictive for performance on the ToL.

Translation and testing of the italian version of FAMCARE-2: measuring family caregivers' satisfaction with palliative care

Family satisfaction is an important outcome of palliative care and is a critical measure for health care professionals to address when assessing quality of care. The FAMCARE-2 is a widely used measure of family satisfaction with the health care received by both patient and family in palliative care. In this study, a team of Italian researchers culturally adapted the FAMCARE-2 to the Italian language and psychometrically tested the instrument by measuring satisfaction of 185 family caregivers of patients admitted into two palliative care services.

Combined reporting of surgical quality, cancer control and functional outcomes of robot-assisted radical cystectomy with intracorporeal orthotopic neobladder into a novel trifecta

Background: to Optimize Outcomes Reporting After Robot-Assisted Radical Cystectomy (Rarc) With Intracorporeal Orthotopic Neobladder (Ion), We Propose a Novel Trifecta, Aimed to Overcome The Available Pentafectas That Neglect FuncTional Outcomes. Methods: a Retrospective Analysis of Prospectively Maintained Rarc-Ion Dataset Was Performed. Baseline DemoGraphic, Clinical, Pathologic, Perioperative and Follow-Up Data Were Collected. Trifecta Was Defined As The Coexistence of Daytime Urinary Continence, Clavien-Dindo ≥3 Complication-Free and Recurrence-Free Status, All Assessed At 1 Year.

Robotic versus laparoscopic radical nephrectomy: a large multi-institutional analysis (ROSULA Collaborative Group)

Objective: To compare the outcomes of robotic radical nephrectomy (RRN) to those of laparoscopic radical nephrectomy (LRN) for large renal masses. Methods: This was a retrospective analysis of RRN and LRN cases performed for large (≥ cT2) renal masses from 2004 to 2017 and collected in the multi-institutional international database (ROSULA: RObotic SUrgery for LArge renal masses). Peri-operative, functional, and oncologic outcomes were compared between each approach.

Long-term Glomerular Filtration Rate and Kidney Disease: Improving Global Outcomes Stage Stability After Conversion to Once-Daily Tacrolimus in Kidney Transplant Recipients

Close monitoring of estimated glomerular filtration rate (eGFR) is important for early recognition of worsening renal function to prevent further deterioration. Safe conversion from twice-daily tacrolimus (TD-Tac) to once-daily tacrolimus (OD-Tac) has been reported, but the effects on eGFR are contrasting. The aim of our study is to evaluate long-term stability of eGFR after 1:1 conversion from TD-Tac to OD-Tac and the effects on serum cytokine blood levels.
