Dienogest versus continuous oral levonorgestrel/EE in patients with endometriosis: what's the best choice?

Combined oral contraceptives (COC) and progestogens are widely used for the treatment of endometriosis. The objective of the study is to compare the efficacy of dienogest 2 mg vs continuous oral levonorgestrel/EE (levonorgestrel 0.1 mg/ethinyl estradiol 0.02 mg) on ovarian endometriomas, deep infiltrating endometriosis (DIE), chronic pelvic pain (CPP), dyspareunia, analgesic use, quality of life (QoL), compliance and side effects.

A ketogenic diet normalizes interictal cortical but not subcortical responsivity in migraineurs

Background: A short ketogenic diet (KD) treatment can prevent migraine attacks and correct excessive cortical response. Here, we aim to prove if the KD-related changes of cortical excitability are primarily due to cerebral cortex activity or are modulated by the brainstem. Methods: Through the stimulation of the right supraorbital division of the trigeminal nerve, we concurrently interictally recorded the nociceptive blink reflex (nBR) and the pain-related evoked potentials (PREP) in 18 migraineurs patients without aura before and after 1-month on KD, while in metabolic ketosis.

Pain chronification. What should a non-pain medicine specialist know?

OBJECTIVE:
Pain is one of the most common reasons for an individual to consult their primary care physician, with the majority of chronic pain being treated in the primary care setting. However, many primary care physicians/non-pain medicine specialists lack enough awareness, education and skills to manage pain patients appropriately, and there is currently no clear, common consensus/formal definition of `pain chronification'.

In clinical practice, analgesia is still empirically monitored by using variation of heart rate, blood pressure, and sweating, as the result of the sympathetic activation, or movements. The challenge of monitoring analgesia in anesthetized patients is related to the interference of hypnosis and cardiovascular drugs used during general anesthesia, and to the rapid changes of the sympathetic system activation observed during surgery.

Tapentadol prolonged release for managing moderate to severe chronic neck pain with or without a neuropathic component

Background: Despite the high prevalence of neck pain, few studies have addressed the pharmacological treatment of this condition. Purpose: We evaluated the effectiveness of tapentadol prolonged-release (PR) in patients with or without a neuropathic pain component, with a focus on functional movements, disability and Quality of Life (QoL). Study design/setting: Observational, retrospective study. Patient sample: Ninety-four adult patients with severe neck pain not responsive to opioid step III treatment.

Sublingual sufentanil tablet system for the management of acute postoperative pain in a hospital setting.An observational study

Background: The use of a strong opioid with intravenous patient-controlled analgesia (IV-PCA) is recommended for postoperative pain, but its use is restricted due to technical problems. Other delivery systems, like sublingual PCA, with the sufentanil tablet system (SSTS) device, could overcome the safety concerns related to IV-PCA.

Methods: This prospective observational study evaluated the efficacy, safety and usability of SSTS for post-surgical analgesia in the real-life setting.

Profiling lasmiditan as a treatment option for migraine

Introduction: In recent years, research into acute migraine treatment has aimed to develop molecules capable of inhibiting trigeminal pathways, mediated by agonism to 5-HT1F receptors in order to avoid the vasoconstrictive action due to the stimulation of 5-HT 1B/1D receptors. A novel migraine drug class, called ‘neurally acting anti-migraine agents’, has been developed for the management of acute migraine attacks. Lasmiditan is the only compound of this drug class that has been evaluated in Phase III clinical trials.
